Job Description:

Under the leadership of the Director of Ambulatory and Enterprise Health System Applications, the Population Health System Manager is responsible for the development life cycle of the healthcare information technology systems supporting the population health program.  This position has a focus on the population health Epic roadmap, implementing Healthy Planet features and functionality, population health analytics, payer contract management, & Ambulatory workflows and quality reporting. The individual in this role will manage the program portfolio and provide direct management of analysts, both employed and contracted, providing oversight to ensure the organization’s population health objectives are met and resources are used effectively.

Role Responsibilities include but are not limited to:

  • Works closely with operational stakeholders to determine strategy & priorities and to ensure the team is meeting business needs
  • Maintain and execute population health Epic roadmap
  • Implement Epic Healthy Planet features and functionality
  • Operationalize existing Epic tools to help increase utilization
  • Help identify additional tools needed to support the context and interventions
  • Coordinate Population Health Steering Committee
  • Other duties as assigned

Manager Responsibilities include but are not limited to:

  • Determining required team resources
  • Managing team staffing
  • Manages, supports, and empowers a quality team of HIT Professionals
  • Acts as a coach and positive role model for staff by establishing & maintaining a safe work environment that fosters positive morale
  • Appropriately monitors and addresses staff performance issues, applying constructive, corrective or disciplinary action when needed
  • Effectively manages staff in compliance with established policies, procedures and legal guidelines
  • Works closely with senior management, stakeholders and Health Information Technology staff to determine strategy & priorities and to ensure the team is meeting business needs
  • Builds and maintains relationships with Montage leaders, external organizations, and vendors
  • Leads or participates in technology related teams, workgroups and committees as needed
  • Engages in strong problem solving and optimization with all necessary stakeholders utilizing best practice methodologies
  • Evaluates factors related to safety, effectiveness, cost, and social impact when developing and implementing applications and or support services
  • Promotes the understanding and effective use of information technology
  • Must be proactive in identifying critical business, technical, and/or operational issues and then drive issues to closure

HIT Division responsibilities include but are not limited to:

  • Maintaining focus on customer service and responsiveness
  • Excellent communication both written and verbal
  • Troubleshoot technical problems
  • Master HIT tools, applications, and processes
  • Maintain fluency in HIT and industry best practices, tools, applications, and processes
  • Follow departmental and organizational policies and procedures
  • Collaborating and working within the division and outside of the division

Experience:
 

Required Experience:

5 years of demonstrated experience including:

  • Using, building, & implementing applications, preferably Epic Healthy Planet
  • Workflow design
  • Project management
  • Excellent verbal and written communication
  • Customer service oriented
  • Knowledge of population health
  • Office 365

Preferred Experience:

Demonstrated experience in

  • Leadership
  • Program Management
  • Developing, implementing and maintaining technology based standard operating procedures
  • Healthcare policies, procedures, and regulatory requirements
  • ITIL practicesIT
  • issue tracking software or "ticketing" systems e.g. ServiceNow

Education:
 

Required:

  • Bachelor’s Degree

Preferred:

  • Master’s Degree in Nursing, Project Management, Computer Sciences, Information Systems, or related field.


Licensure/Certification:

Required:

Epic Healthy Planet certification (current)

Desired:

  • Epic certification in multiple modules
  • Project Management Professional (PMP)
  • Professional training in LEAN process methodologies
  • ITIL Certification
  • Change Management
