Job Description:

The Nurse Practitioner is responsible for providing assessments of hospice patients in their homes or facilities, and assisting the hospice Medical Director in determining continued hospice Medicare-benefit eligibility. The Nurse Practitioner will gather clinical findings and will provide that information to the physician who then re-certifies the patient for continued hospice care, if appropriate. Duties include patient assessment; evaluations; performing physical exams and appropriate tests; initiating, renewing, and changing medications; and charting results. The Nurse Practitioner must demonstrate excellence in patient care, observation, assessment, nursing judgment and communication skills. Must demonstrate ability in the areas of interpersonal relations, critical thinking, problem solving, decision-making skills and working independently.

Experience:

A minimum of 2 years experience as a Nurse Practitioner is required. Nurse Practitioner experience specializing in Geriatric or Hospice care preferred. The understanding of hospice philosophy and principles of death/dying is also highly preferred.

Education:

Level of education sufficient to obtain CA RN license and CA Nurse Practitioner license.

Licensure/Certification:

Must have a valid CA RN and CA Nurse Practitioner license, and current American Heart Association Healthcare Provider BLS certification.

Other:

This position requires driving. In addition to a valid CA driver’s license, the following are required: Automobile insurance as required by the State of California with an acceptable driving record of three points or less, participation in the DMV pull-notice program, and automobile available for daily work.
