Position Summary

We are looking for a Health Coach, bilingual in Spanish and English, to work directly with members of our managed care contracts and the greater Monterey County community to improve health outcomes by providing disease management and wellbeing services utilizing evidence-based health behavior-change strategies. The Health Coach is responsible for identifying condition specific lifestyle modification opportunities, establishing goals and action plans, and working collaboratively with internal and external multidisciplinary teams to provide necessary support and resources to the diverse populations we serve. Responsibilities

  • Employ behavior change/motivational interviewing skills to assess readiness for change, facilitate long and short term goal setting, address barriers to change, and provide appropriate health education aligned with clinical guidelines and national evidenced-based criteria
  • Facilitate group coaching and education classes to support understanding of disease prevention, disease management, and condition-specific lifestyle management strategies for diverse populations
  • Be accountable for tracking, reporting, and effectively improving health outcomes of your patient caseload
  • Support patient outreach, engagement, and enrollment efforts for Aspire Health wellness programs and population health needs
  • Collaborate with Aspire Health team members, physician offices, and community partners to address patient needs and coordinate appropriate resources to support health self-management
  • Document following standard workflows and run reports to identify, action, and monitor patients within the Electronic Medical Record (EMR) and other technology tools
  • Design, innovate, implement and evaluate curriculum by request of internal and external partners
  • Other duties as assigned.

Position Requirements

  • Bilingual English/Spanish
  • Bachelor’s degree in a health related field (i.e. health promotion, public health, nutrition, kinesiology, etc.)
  • At least two years of experience providing impactful health promotion or disease management services (clinical setting preferred)
  • Knowledge of managed care concepts, health promotion strategies, motivational interviewing, transtheoretical model and other behavior change tools
  • Strong commitment to teamwork with ability to contribute expertise and follow leadership directives
  • Ability to thrive in a continually evolving environment
  • Strong verbal and written communication skills
  • Strong organizational and prioritization skills
  • Strong proficiency in Windows-based database applications
  • Ability to work independently and manage a large caseload with minimal supervision
  • Exceptional problem solving and customer service skills
  • Ability to effectively establish rapport with patients, providers, and stakeholders in person and via telephone and/or online modalities

Preferred Qualifications

  • Health Coach certification
  • ACSM, CHES, or related certification
  • NDPP Lifestyle Coach certification
  • Experience working in a healthcare setting and/or documenting in electronic medical records
