Job Description:

The Family Care Manager (FCM) reports to the Program Manager of Family Care Management and is responsible for assessing the social needs of child and adolescent patients and their families when they present with complex psychiatric and related psycho-social problems. FCM provides information, parent guidance, emotional and practical support to families to enhance functioning and further treatment goals.

 

The FCM assists patients and their families in understanding and managing issues that may be affecting their behavioral health status, including legal, educational, financial, occupational, transportation and insurance issues. The FCM will facilitate referral to services and programs outside of Ohana when indicated. As part of their role, the FCM will engage in ongoing efforts to learn about effective local and regional resources for the families of Ohana patients and will build collaborative relationships with the programs and institutions providing these services. The FCM will assist with psychoeducation on an individual level as well as through groups and workshops. As a member of a multidisciplinary mental health care team, FCM will collaborate with Ohana colleagues to provide appropriate support to patients and families. This will include attending clinical meetings and working closely with therapists, psychiatrists, and other behavioral health staff. Unlicensed FCM are expected to work towards licensure. Finally, the FCM will fulfill other necessary tasks as needed to support patient care at Ohana as assigned by their manager or the leadership team.

The Family Care Manager job family consists of three levels of professional, non-management roles: Unlicensed Family Care Manager, Licensed Family Care Manager, and Senior Family Care Manager.

 

 

Experience:

Must have a minimum of 3 years’ experience working with children, adolescents or families and knowledge of the local and regional resources that are critical to the families of youth with behavioral health conditions, such as the educational system, legal resources, juvenile justice system, community services, and public insurance (MediCal) programs.

 

Education:

Master’s degree in Social Work or related area (Psychology or Marriage and Family Therapy) is required.

 

Licensure/Certifications:

Must have begun the process of registering to become an Associate with the BBS. American Heart Association Healthcare Provider BLS certification required. Will complete mandatory 5150 training by Monterey County to obtain 5150 certification and obtain 5150 Professional staff authorization yearly.
